Installed Updates Reporting 'Is Required'

Just when I thought I was getting the hang of SCCM 2012, I am having issues with an update that shows as installed in Programs and Features, but is still reporting in SCCM as Is Required.  The client last reported date is recent and well past the date installed shown in Programs and Features.  I've looked through SCCM logs via SCCM Support Center, and could use some guidance as to what I should be looking for.
